NAD+ rate-sensitive — 250 mg over ≥ 2 hr first session (90–120 min if tolerant); 500 mg over ≥ 3–4 hr. Start 2 mg/min, titrate to 4–5 mg/min only if asymptomatic at 30 min. Dedicated bag only — never co-mixed.
Magnesium — infuse the Mg portion over ≥ 15 min minimum (rapid push → flushing, hypotension, AV block). Calcium Gluconate 1 g IV slow is the antidote for Mg toxicity.
Vitamin C — ≤ 0.5 g/min for 5–10 g. CARD-VC verbal screen required for 6.1–10 g (incl. G6PD); G6PD-deficient → cap 2 g or decline. > 10 g out of wellness scope.
Glutathione IV push only — post-drip, 1–2 min, via Y-port after line flush. Never in the bag, never with Vit C (redox), never piggyback/slow infusion. Ceiling 1,200 mg.
Calcium — Gluconate preferred peripherally; never in the Vit C bag (precipitate). Hold if digoxin/cardiac glycoside. Calcium Chloride is a vesicant — central preferred.
GFE Gate-0 — valid GFE (< 12-mo), patient cleared, standing orders authorize the act, FORM-02 done this visit — before chair assignment or compounding.
